Submit Concussion Report

At the request of the IHSA Sports Medicine Advisory Committee (SMAC), the IHSA is asking all member schools to submit reports on student-athletes who have sustained a concussion. If your school has or uses the services of a certified athletic trainer (ATC), it is most appropriate for him or her to fill out this report. However, if your school does not have or utilize an ATC, then our office would request that a school administrator (Principal or AD) complete this report for recording purposes. The SMAC will review the data throughout the year to monitor any trends that may develop; all reporting will be anonymous as it relates to student names.
